Lichen planus is a non-infectious, itchy rash that can affect many areas of the body.
Affected areas can include:
- the arms and legs
- the mouth (oral lichen planus)
- the nails and scalp
- the vulva, vagina and penis
Although lichen planus can present with a variety of lesions, the most common presentation is as a well-defined area of purple-coloured, itchy, flat-topped papules with interspersed lacy white lines (Wickham’s striae). This description is known as the characteristic “6 P’s” of lichen planus: planar (flat-topped), purple, polygonal, pruritic, papules, and plaques.
Pathology of Lichen Planus-
Scanning power view of lichen planus shows a lichenoid reaction pattern characterised by the combination of degeneration of the basal layer of the epidermis and a band like lymphocytic infiltrate obscuring the dermoepidermal junction. There is irregular epidermal hyperplasia forming a characteristic saw-tooth appearance with wedge-shaped hypergranulosis . The basal layer of the epidermis exhibits vacuolar degeneration with typically prominent necrosis of individual keratinocytes. The inflammatory infiltrate is chiefly lymphocytic and forms a dense band in the superficial dermis.
Causes of Lichen Planus-
-The cause of lichen planus is unknown. In certain locations, patients with extensive lichen planus seem to be more likely to have a hepatitis C virus infection of the liver. However, it is unclear if this virus is the cause of lichen planus in such situations.
-Some drugs, such as those containing arsenic, bismuth, or gold, can produce an eruption which appears identical to lichen planus.
-Exposure to certain chemicals used in the development of color photographs can also produce a similar rash. The long-term use of the drugs quinacrine orquinidine (Quinidine Gluconate, Quinidine Sulfate), which is used formalaria.
-Certain microscopic organisms, and worms, may produce hypertrophic lichen planus of the lower legs and other skin and body-wide (systemic) disturbances.
-Lichenoid eruptions can occur in graft-versus-host disease in people who have received bone marrow transplants.
Types of Lichen Planus-
Lichen planus may be categorized as affecting mucosal or cutaneous surfaces.
- Cutaneousforms are those affecting the skin, scalp, and nails.
- Mucosalforms are those affecting the lining of the gastrointestinal tract (mouth, pharynx, esophagus, stomach, anus), larynx, and other mucosal surfaces including the genitals, peritoneum, ears, nose, bladder and conjunctiva of the eyes.
Annular- Ring-shaped’ lesions that develop gradually from single small pigmented spots into circular groups of papules with clear, unaffected skin in the center. Annular lesions occur in approximately ten percent of lichen planus cases. The ring-like lesions may very slowly enlarge, co-join and morph into larger irregular (serpentine) bands, sometimes accompanied by lines.
Linear-Papules are arranged in a line (the “Blaschko line”).This pattern may develop secondary to trauma (koebnerization) or, uncommonly, as a spontaneous, isolated eruption, usually on the extremities, and rarely on the face.
Hypertrophic- This pattern usually occurs on the extremities, especially the shins and the interphalangeal joints, and tends to be the most pruritic variant of lichen planus. Also known as “Lichen planus verrucosus”.
Atrophic- This morphology is characterized by the presence of a few well-demarcated, white-bluish papules or plaques with central superficial atrophy. This is a rare variant of lichen planus.
Bullous- This morphology is characterized by the development of vesicles and bullae with the skin lesions. This is a rare variant of lichen planus, and also known as “Vesiculobullous lichen planus”.
Ulcerative- This morphology is characterized by chronic, painful bullae and ulceration of the feet, often with cicatricial sequelae evident. This is a rare variant of lichen planus.
Pigmented- This morphology is characterized by hyperpigmented, dark-brown macules in sun-exposed areas and flexural folds. This is a rare variant of lichen planus.
Complication of Lichen Planus-
Lichen planus can be difficult to manage on the vulva and in the vagina, causing severe pain and sometimes leaving scars. Sexual dysfunction can become a long-term complication.
There is some evidence that lichen planus may increase the risk of a skin cancer known as squamous cell carcinoma, though the risk is small.
Treatment of Lichen Planus-
Corticosteroids may reduce inflammation associated with lichen planus.
Retinoids are synthetic versions of vitamin A that can be applied topically or taken orally for treatment.
Nonsteroidal creams or ointments
Topical calcineurin inhibitors reduce immune-system activity involved in lichen planus and appear to be particularly helpful in managing lichen planus of mucous membranes like tacrolimus (Protopic) and pimecrolimus (Elidel).
Antihistamines act against a protein called histamine that is involved in inflammation. An oral or topical antihistamine may relieve itching or pain associated with lichen planus.
Light therapy, or phototherapy, may help clear up lichen planus affecting the skin. The most common phototherapy for lichen planus uses ultraviolet B (UVB) light, which penetrates only the upper layer of skin (epidermis). There is some risk of “sunburn” with this treatment.

Thuja – Good for inveterate skin troubles and chronic stubborn skin conditions like lichen planus, psoriasis, eczema, warts, pyoderma, etc. Tendency to have unhealthy skin in general. These patients are better in dry weather and worse from humid climate and damp conditions.
Arsenicum iodatum – It is a very good remedy for skin conditions like lichen planus, eczema and psoriasis where there is marked itching and exfoliation of the skin. There is exfoliation of large scales which leaves behind raw exuding surface.
Ignatia-Itching, nettle-rash. Very sensitive to draught of air. Excoriation, especially around vagina and mouth.
