Molluscum contagiosum is a viral skin infection that causes either single or multiple raised, pearl-like bumps (papules) on the skin. It is a chronic infection and lesions may persist from a few months to a few years.

Pathology of Molluscum contagiosum-

In molluscum contagiosum, there is usually pronounced infundibular hyperplasia and papillomatosis with central umbilication.

Millions of virions that have proliferated in the cytoplasm of affected epithelial cells result in the characteristic intracytoplasmic bodies that compress the keratinocyte nucleus. This viral inclusion is the largest in all of human histopathology.

Follicular rupture may occur resulting in extensive folliculitis, suppuration and inflammatory changes. In such cases, the viral inclusions may be difficult to find.

Complication of Molluscum contagiosum-

The bumps and the skin around them may become red and inflamed. This is thought to be an immune response to the infection. If scratched, these bumps can become infected. If lesions appear on the eyelids, pinkeye (conjunctivitis) can develop.

Surgery

Surgical treatments include cryosurgery, in which liquid nitrogen is used to freeze and destroy lesions, as well as scraping them off with a curette. Application of liquid nitrogen may cause burning or stinging at the treated site, which may persist for a few minutes after the treatment. Scarring or loss of color can complicate both these treatments. With liquid nitrogen, a blister may form at the treatment site, but it will slough off in two to four weeks. Although its use is banned by the FDA in the United States in its pure, undiluted form, the topical blistering agent cantharidin can be effective. Cryosurgery and curette scraping are not painless procedures. They may also leave scars and/or permanent white (depigmented) marks.
